Keep Connected Support Coordination is led by Bianca Phelan who has worked with people with disabilities for more than 10 years in various different roles such as youth worker, support worker, teachers aid and working alongside Child Protection. Bianca created Keep Connected Support Coordination to meet the growing number of participants in South East Melbourne who require Support Connection, Coordination and Specialist Support Coordination Services. 

Bianca's specialties lie with people requiring support with autism, intellectual disabilities, deaf and hard of hearing, and psychosocial disabilities. However Bianca has experience working with a diverse range of disabilities and can cater to anyone.

Keep Connected Support Coordination takes a person centered approach to each participant and empowers them to achieve their NDIS goals. We lead a holistic approach and can assist with formal and community supports as well as informal supports such as building and maintaining relationships with family and friends.

For the last 5 years, Bianca has worked alongside local stakeholders including Child Protection to support incredibly complex people to achieve their goals. Intricate cases are welcomed.
