Keep Connected Support Coordination (KCSC) is led by Bianca Phelan, she created KCSC to meet the growing number of participants in Southeast Melbourne who require Support Coordination and Specialist Support Coordination Services.
Bianca has assembled a highly effective and motivated team of Support Coordinators ready to help you.
Bianca has a Diploma of Community Services and has worked with people with disabilities for more than 15 years in varied roles such as youth case management, disability support worker, teachers aid in specialist disability schools, working at the DFFH to build funding packages and now Support Coordination.
Rebecca holds a Diploma of Community Services and has over 20 years working in the Community services sector. She has many years supporting youth in case management roles, targeted care packaging and working at the DFFH.
Bec is the most senior employee of KCSC and holds the role of Specialist Support Coordinator.
Amelia has completed a Diploma in Mental Health and is working towards a Bachelors of Social Work and a Graduate Certificate in Family and Domestic Violence Practices. Amelia has been working in the industry for a decade and has held roles across Disability and Out of Home Care both in support work and case management.
Katherine has a Diploma in Community Services and had extensive work experience as a support worker and Forensic residential support worker before transitioning into Support Coordination.
Rachel holds a Bachelor of Psychological Science with a Master of Profession Psychology which she used to work as a Provisional Psychologist supporting youth in Custody.
Rachel has transferred these skills and experience to support people on the NDIS.
Michelle has a Cert III in Community Services, her previous experience as an NDIS Plan manager gives her special insight and knowledge when navigating the NDIS for participants.
KCSC takes a person centered approach to each participant and empowers them to achieve their NDIS goals. We lead a holistic approach and can assist with formal and community supports as well as informal supports such as building and maintaining relationships with family and friends.
